Discrimination is against the law.
Legacy Care Hospice complies with all applicable Federal civil rights laws and does not discriminate, exclude, or treat anyone unfairly based on race, color, religion, national origin (including limited English proficiency and primary language), sex (including sex characteristics, intersex traits, pregnancy or related conditions, sexual orientation, gender identity, and sex stereotypes), age, disability (including mental health conditions and substance use disorders), or any combination of these factors in admission, access to services, treatment, or employment.
We provide reasonable accommodations to individuals with disabilities, including auxiliary aids and services needed for effective communication. These may include qualified sign language interpreters or information provided in alternative formats such as large print, audio, or accessible electronic formats. We also offer free language assistance services to individuals whose primary language is not English, which may include qualified interpreters and translated materials.
For requests related to reasonable modifications, auxiliary aids and services, or language assistance, please contact our Civil Rights Coordinator.
If you believe that Legacy Care Hospice has failed to provide these services or has discriminated in any other way, you may file a grievance with our Civil Rights Coordinator. You can do so by phone or mail by contacting your local regional office listed on this website and requesting to speak with the Civil Rights Coordinator. You may also email us at info@legacycarehospice.com. If you need help filing a grievance, your local Civil Rights Coordinator is available to assist you.
